About Long-Term Care Health Facilities

This facility provides elderly people with relatively stable medical conditions with care, nursing care, and physical function recovery training, and helps them to return to their homes as soon as possible while maintaining ties with their homes and communities.

About this facility

We aim to be a "Loved and loved facility" for users, families and local residents.
We provide an environment where people who need all-around care and nursing care can receive high-quality all-around care, nursing care and rehabilitation with peace of mind. This facility has three floors, 152 beds, and a day care floor, and supports the recovery of users by providing all-around care, nursing care, and help with physical functions.
We provide daily education and training for staff. The staff working at this facility undergo daily education and training so that, as professionals, they can provide support based on the wishes and goals of each user.
At the same time, through all-around care, nursing care, rehabilitation, and other activities, we provide a cheerful, family-like atmosphere so that people can feel relaxed and reassured. In addition, we work in conjunction with local activities, volunteer activities, and cooperating hospitals and dentists to ensure that health, medical care, and welfare are an integral part of the community.

■Features

  • We provide all-around care and nursing care under medical management by doctors.
  • We provide all-around care and nursing care 24 hours a day, 365 days a year.
  • In order to create a more family-like atmosphere, we proactively adopt unit care.
  • In order to make help users return home as soon as possible we conduct frequent physical therapy sessions.
  • A speech-language-hearing therapist (as part of physical therapy) and a registered dietitian work together to help users eat meals, instead of using tube nutrition or other means.
  • A music therapy program is performed by a music therapist
  • We put effort into environmental management of the facility, infection control, and complete elimination of odors.
  • We proactively hold seasonal events and activities that provide opportunities for interaction with the local community
    (Royal Athletic Meet, Summer Festival, Royal Festival, Christmas Party, Setsubun, Orange Cafe)

■In the case of long-term residence

Those who are certified as requiring long-term care level 1 ~ 5

■In case of short-term residence and outpatient use

Persons certified as requiring support or requiring long-term care 1 ~ 5
z There is a transportation service by car for those who use Short-Term Residence for Recuperation and Outpatient Rehabilitation.

Description of services provided

■For residential users

We create an all-around care plan for each individual patient, and provide services such as medical services provided by a doctor based on the patient's symptoms , 24-hour nursing care by a nurse, living assistance such as feeding and bathing, rehabilitation three to six times a week, daily recreation, and seasonal events.
During long-term residence, the care manager, counselor and staff in charge of the facility meet regularly with the users and their families. We provide in-home support according to the symptoms and wishes of the users.

■Short-term residence

Our services are available when temporary care is needed for social or personal reasons such as family care, illness, or ceremonial occasions.
Transportation to and from the facility can be provided upon request.
Short-term residence is available from one day to 30 days.

Medical services by doctors, nursing care by nurses, all-around care, rehabilitation and seasonal events are available during the period of use.

■Outpatient

This is a daytime that provides transportation in the morning and evening.
After consultation with a doctor, services such as bathing (everyday-living services), meals (nutrition guidance), rehabilitation and recreational services are provided under the appropriate guidance of professional staff.
Transportation can be provided for wheelchair users. .
This service is aimed at helping to establish a regular daily rhythm for users.
Day care is provided six times a week, on Monday through Saturday.
Users can select the day of the week and time according to their preferences.
*The number of times the service can be made available, and the duration of use, depend on the user’s degree of certification and the care plan.

■Access from the train station

  • Get off at Dokkyo Daigaku-mae Station on the Tobu Isesaki Line.
    10-minute walk from the Sogo Ground bus stop on the Tobu bus bound for Shinden Station East Exit.
  • • Get off at Shinden Station on the Tobu Isesaki Line.
    Take the Tobu bus (located near the east exit of Shinden Station going in the direction of Dokkyo Daigaku-mae Station). Get off at the “Sogo Ground” stop. From there, it is a 10-minute walk to the facility.
    You can also take the bus going to the Kakinoki Community Center and get off at “Shimin Onsui Puru Iriguchi”. From there, it is a 5-minute walk to the facility.■map of the surrounding area
